Can I use Puffy Foam directly on fabric for use as an applique?

It will not hold up as a stand-alone "fabric". Once it's perforated by the needle, unless it is enclosed in stitching, it will break away, as it is intended to do. So it needs to be encased either by fabric completely or by stitching.
