Hmmm, I didn’t have this problem (as you know). Size 80/12 Microtex Needle.
Did she use the recommended 3mm stitch length for topstitching?
What did she use in bobbin?
The four layers are the strap parts. So was this problem only when stitching those short ends together around the D-rings? Or did it happen throughout the construction?
Did she try going up to a size 90/14 needle to prevent the shredding? I cannot think of why this happened.
Hopefully these answers will help us figure out what happened. Shredding at the needle almost always indicates a too-small (or damaged) needle; or...a too tight top tension. There can be other issues like a burr on the bobbin or throat plate.
